Admission/Visitors’ Guide

[No visitors are allowed due to the COVID-19 pandemic.]

  • To prevent the spread of hospital infections, no visitors are currently allowed at the Fukui Prefectural Hospital as a general rule.
  • We cordially ask your understanding and cooperation in order to protect yourself as well as your family.
  • Visits are allowed under the following conditions only with a “Visit Certificate” given at the time of admission.<
    1. The visit is requested by the hospital: Maximum of two (2) persons
    2. For delivering laundry: One (1) person only
  • A visit is limited to once a day, for 10 minutes at maximum.
  • Visitors from Tokyo prefecture and from the “Spread of infection alert areas” are NOT allowed, in principle.

  • * Visitors from outside the prefecture may visit after the elapse of two weeks after their arrival, and if they show no symptoms such as fever. A doctor’s permit is still required for a visit.
  • Any visitor with a body temperature higher than 37℃ detected at the hospital entrance will be denied entry to the hospital.
  • Please ensure to wear a facemask and observe social distancing of more than one meter.
  • The hospital staff members are not allowed to accept any delivery of personal goods to a patient.

Hospitalization procedures (except for urgent hospitalization)

Hospitalization procedure

(1) When a patient’s hospitalization is scheduled, the patient proceeds to do paperwork at the General Reception Counter A on the 1st floor.
(2) On the hospitalization day
The patient submits the following documents at the 1F General Reception Counter A.
(All patients)

  • Hospitalization Application Form
  • Letter of Consent (agreeing to pay the actual cost of medical fees)
  • Hospitalization Guarantee Form (filled out by a guardian of the patient or cosigner with his/her signature and seal. A cosigner should be a person who has separate livelihood from the patient.)
  • Patient Registration Card
  • Health Insurance Card

(Applicable patients)

  • Certificate of Eligibility for Ceiling-Amount Application (See below)
  • Claimant Certification for Medical Care (Children’s Medical Care, Medical Card for Severely Handicapped, Maternal and Child Medical Care)
  • Claimant Certification for Public Expense (intractable diseases, specified diseases, medical and infant care services)
  • Discharge Certificate (if patient had been hospitalized in the past three months)

(3) Please go to the Emergency Medical Center to have the admission procedure during after-hours and on weekends.
(4) Please contact the outpatient reception of each department in case a patient cannot be hospitalized for any compelling reason or cannot come to the hospital on time.
(5) Please immediately inform the administration staff or nurse in the ward of any changes in the following matters during the hospitalization.

  • Change in the type of health insurance due to new employment or retirement
  • Expiration of the health insurance
  • Change of address

* Please make sure to submit your health insurance card in order to avoid out-of-pocket payment for the full amount.

Please do not hesitate to ask any administration staff member in the ward if you have any questions.

<Certificate of Eligibility for Ceiling-Amount Application>

On presentation of the Certificate of Eligibility for Ceiling-Amount Application, a hospitalization copayment of a patient who is 69 years old or younger will be reduced to the ceiling amount per month according to the patient’s income bracket. Apply for the Certificate of Eligibility for Ceiling-Amount Application with your health insurer (*) beforehand. Present it to the administrative staff member in the ward.

*Other health insurance (Japan Health Insurance Association, Mutual aid, Union, etc.)
Your insurer or employer indicated on your health insurance card

Please make inquiries to a ward staff member if you have any questions.

What you need during hospitalization

Please bring the following items when you are hospitalized.

  1. Patient registration card and your Seal
  2. Commodities including:
    Face towel, bath towel, underwear, tissue paper, trash box, footwear (with your heels covered), etc.
  3. Toiletries including:
    Toothbrush, toothpaste, soap, shampoo, shaver (razors are strictly prohibited), comb, etc.
  4. Meal gadgets/utensils including:
    Chopsticks, spoon, teacup, small kettle
  5. Prescription drugs
    If you are currently taking any medication, please bring the medicine as well as information on the medicine (medicine bag, instruction manual, drug notebook, etc.).
    For details, please refer to the “Instruction on how to submit your prescription drugs” that is given to you when you apply for hospitalization.

  7. Others
    • What you need may be slightly different depending on the hospital department. An explanation will be given at the orientation.
    • Please bring as little as possible and ensure to put your name on each item.
    • Nightwear (pajamas)・・・・We ask patients to wear the hospital-designated nightwear (rental for 70 yen/day), in principle. If you need to wear your own nightwear out of necessity, please bring one with you.
    • For infant patients, please bring your own milk and paper diapers. The water server in the Dining/Day Room is available for making baby’s milk.

Please note that the following items are prohibited in the hospital.

  • Hazardous items (scissors, nail clippers, knives, etc.)
    Please bring an electric shaver instead of a razor.
    Please ask a nurse when you need scissors and/or nail clippers.
  • Alcohols

Discharge procedure

  1. When the patient has received permission for discharge from his/her doctor, he/she will be given instructions by a nurse as to precautions after discharge and follow-up visits.
  2. Please completely settle the medical bill for the hospitalization before you leave the hospital. Make sure to receive the medicine and documents after the payment is confirmed.
  3. We ask patients to complete the discharge procedure before noon.
  4. When you visit for follow-up exams after discharge, please remember to bring your health insurance card for our review once a month. Make sure to take all your belongings with you when you leave the hospital.

About Visiting

To visitors:

Please make sure to visit a patient within the visiting hours, scheduled specifically to avoid interference with doctors’ consultation hours as well as to ensure enough time for complete rest and recovery for the patient. However, please understand that we may ask you to refrain from visiting even within the visiting hours depending on the patient’s condition.

Visitors’ Guide

Visitors may enter the hospital from the front entrance or from the after-hours entrance (North side). Please note that the front entrance is closed (locked) at 6 p.m. on weekdays. Visitors can come and visit patients freely.

In some cases, however, visitation may be limited according to the patient’s request or doctor’s instruction, or denied due to medical exam scheduling or for other reasons.

Visitors are not allowed to see patients after visiting hours. A large group of visitors is also not allowed. However, if there is any special reason, please let us know. Try not to stay too long and observe silence out of consideration for other patients.

Please refrain from visiting while you are affected by any epidemic disease and from visiting with small children because of the risk of infection as well as out of consideration for other patients.

ICU, Emergency Medical Center, Maternal and Child Medical Center, Clean Room (12 North Ward)

There are special rules for these facilities. Please ask a nurse about the details.

Use of cellphones

  • Please consult a hospital staff member about the use of cellphones in the hospital, which may affect the medical devices.
  • Please use the cellphone in silent mode.
  • Please use the cellphone with consideration for other patients. Please step outside the room if you are in one of the large rooms. Please be sensitive especially after 9 p.m. so as not to disturb the sleep of other patients.
  • We ask all patients to follow the instructions of the hospital staff members for medical safety.

Protection of privacy in relation to inquiries about hospitalization

The hospital has the right to decline any inquiries about a patient’s hospitalization or about a patient’s room number or any request for a visit by someone in order to protect the privacy of the patient. If you wish to have your privacy protected, please notify a nurse when you are admitted to the hospital so that for any inquiries about you on the phone or in person, we will answer, “That patient is not hospitalized.”
